A Registration and Health History form must be completed for each camper. You can download the forms in PDF format, or as a Microsoft Word document. Mail the completed form with a check to:

Also, if any medication (prescription or over the counter) needs to be given while at camp, the Medication Permission form needs to be completed by your physician as well as a parent/guardian. This form along with your child’s immunization record can be sent at a later date (but at least by the first day of camp) via fax (439-3872)
